The mdt/ost index is recorded in the MGT, so what I suggest is

1. reformat MGT with mkfs.lustre --reformat
2. tunefs.lustre --erase-param on all MDTs and OSTs
3. tunefs.lustre --writeconf on all MDTs and OSTs with appropriate mgt/mdt
parameters

This will  revive your metadata and object data.
